Athletic Training Participation Forms

GCSU Athletic Department Athletic Training
Forms Required for Athletic Participation: All forms, apart from the Tryout Waiver, are required of all athletes (all squads competing for Peach Belt Championships, including intercollegiate teams, cheerleaders, and dance team). Student-athletes are not eligible for participation or covered by the Athletic Department's insurance policy until successfully completing a preseason physical exam. Those athletes who desire to try out with a team must pass a physical exam by a physician and complete the Tryout Waiver PRIOR to the scheduled tryout session.
Preseason Physical	Required for all student-athletes accepted to your squad; Athletes will not be covered on the Athletic Department's insurance plan without a physical. Must be completed by a physician (DO or MD)
Consent to Treat/Assumption of Risk/Release of Medical Information	Provides consent of the athlete to be evaluated and treated by the GCSU sports medicine staff Provides Assumption of Risk notification to the athlete Provides for release of relevant medical information to the GCSU sports medicine team and GCSU Athletics personnel as necessary. Not required for tryouts.
Insurance Information	Required for all student-athletes accepted to your squad. Provides contact information for the athlete and parents/guardians. Provides information about the athlete's personal insurance coverage Not required for tryouts
Tryout Waiver	Must be completed PRIOR to tryout with your team and kept on file in the Training Room. Acknowledges the financial responsibility of the athlete (and not GCSU) and releases the university from any liability in the event of an injury during the tryout. Preseason physical exam is required for the tryout.
